Zero-doses Children by percentage of children population across years
In 2022, the number of surviving children (aged 12 to 23 months) who have never received a single dose of vaccine, known as zero-dose children, was estimated at 7.7 million (20% of surviving children), compared to 7.6 million (20% of surviving children) in 2021, 7.1 million (19% of surviving children) in 2020, and 6.2 million (17% of surviving children) in 2019. Figure presents the distribution of the number of zero-dose children and under-immunized children (dropout for DTP1) in the African Region per year. The under-immunized children accounted for 7% of surviving children in 2022 compared to 8% in 2021.
Cumulatively, the number of zero-dose children from 2019 to 2022 in the African Region is estimated at 28.7 million children, accounting for 19.0% of the four cohorts of surviving children.
High Zero-Dose Rates AND High Burden:
A Dual Crisis Countries like Equatorial Guinea, and Chad show zero-dose rates above 60–80%, and have large-sized bubbles, meaning a high absolute number of children missed.
These are high-priority countries for immunization recovery and equity-focused campaigns.
The Giants:
Nigeria, Ethiopia, and DRC Nigeria has one of the largest bubbles despite a moderate zero-dose rate (~40–50%). This reflects the sheer population size and requires intensive outreach, especially in rural or insecure regions.
DRC and Ethiopia also show large zero-dose numbers, stressing the need for supply chain strengthening, routine immunization, and community trust-building.
Outliers:
Low Coverage, Low Numbers Some countries like São Tomé and Príncipe or Eswatini show low DPT1 coverage, but have small populations, leading to smaller bubble sizes.
These are ideal for targeted microplanning and rapid recovery efforts.
Success Stories
Countries like Rwanda, Botswana, Burundi, and South Africa appear with low zero-dose rates and small bubble sizes.
They showcase effective national immunization programs and can serve as regional models for best practices.
